Authorize Grooming Centre to collect, record, use, store, process all information relating to me and my transactions with Grooming Centre on the Smartbiz Service.

To ensure that I understand how to properly use the applications and software required for the Smartbiz Service, including any upgrade or changes to the Service.

I shall be solely responsible for the safe-keeping and confidentiality of the PIN, User ID and Passwords relating to the Smartbiz Service, and the Statement of Transactions and Balances obtained from the Smartbiz Service.

That any disagreement with entries on my Statement of Transactions and Balances on the Smartbiz Service shall be made to Grooming Centre Clients’ Services through info@groomingcentre.org within 14 days of such transaction

That Grooming Centre shall not be liable for any failure to perform any obligation as contained in this Consent Form or for any loss or damage whatsoever suffered or incurred by the me howsoever caused and whether such loss or damage is attributable (directly or indirectly) to any dispute or any other matter or circumstances whatsoever.

That if I wish to withdraw my consent or have concerns relating to the processing of my personal information, I may do so at any time by notifying Grooming Centre through info@groomingcentre.org and Grooming Centre shall respond within 30 days of receiving my notice.
